We have an open list and welcome requests for registration from patients living in or moving to the practice area.
To register, please collect a registration pack from the surgery.
To enable the practice to register you correctly, we will need to establish that you are ordinarily resident in the UK on a settled basis. Each person registering must complete a registration form and produce two forms of identification.
We accept the following forms of ID:
A valid passport, birth certificate, valid UK photographic driving licence, current NHS card, PLUS a recent utility bill or bank statement etc, showing your name and current address.
- If you are aged 18 or over, you will need to make an appointment for a New Patient Check with one of our Health Care Assistants.
- If you are on repeat medication, you will need to produce a copy of your repeat medication list at the time of registration which will be reviewed by a GP.
- All pregnant women and patients on oral contraception will need to make an appointment to see a nurse.
- Patient that fail to attend booked appointment(s) without notifying the practice may have their registration request refused and asked to register elsewhere.
